Clients Rights and Complaints

Your experience and satisfaction with services are important. If you have a concern about the services you received, you have several options available. You may choose to complete one or more of the steps below:

1. Request Your Health Care Records

You have the right to request a copy of your health care records directly from the licensee (provider). To request records, please contact our office. You will have to complete a written authorization form and will be asked to pay a reasonable fee for copies, consistent with applicable laws and regulations.

2. Contact the Texas Behavioral Health Executive Council (BHEC)

3. File a Consumer Complaint with the Texas Office of the Attorney General
